Physicals Required for Marching 2015-2016!

Please note that physicals are now required for the upcoming marching season, and your student must have the packet completed by May 29th.  That gives us only 2 weeks!  If you have any questions at all, please contact Marie Slany in the Activities office.  She was just recently informed of the change and is waiting to approve the Marching Students.  

Here is a link to the Activities page with all the documents necessary:  Physical Packets  Please note that not all forms are needed.  I've also put the necessary forms in our archive here: Forms and Archive, but please check with the activities office if you are unsure or have any questions.

If you are unable to make an appointment with your regular doctor, there are many places which do school physical exams such as CVS or NextCare.

Please don't put this off!  We want to start the year off in full force! 

Parents, see below for the low-down:

There are 13 points on the checklist, you only print forms #5, #6 (pgs 1-3), #7 (pg 1), #8, and #11.

#1:  $50 check is required for Band Fees, which is due at time of book pick-up.

#2 & #3: These are for NEW STUDENTS only

#4:  Read before completing the packet

#5:  Print this page and fill it out.  It must be signed in front of a school official or notarized.

#6: Print pages 1 - 3 unless you want a map of the NextCare Clinics.  You must complete all 3 pages and have the physician sign at the end.

#7: Print page 1 only unless you want a map of the NextCare Clinics.  The physician must complete this form.

#8: Print this form and fill it out with the student.

#11:  Print and fill out.
